Complex Care Coordinator

As a Care Coordinator (or Case Manager) for her clients, Anita seeks to remove the struggle of the everyday routines, eliminate challenges and boost a client’s independence.  Importantly, Anita also understands the value of companionship.  It is so important that a client experience the feeling of not just being wanted, but also needed.   Some of the Care Coordination duties that Anita will provide to her clients include:

  1. Assisting with personal care: Anita will assist in selecting a direct personal caregiver if that role has not been filled. Additionally, Anita will coordinate between the family, caregiver, medical professionals as well as any other needs that are deemed necessary by the family. Anita has worked in the role of direct personal caregiver for many years and can fill in as needed. These task include bathing and grooming, dressing, toileting, and light exercise.   If bed-bound Anita can supervise the allowed exercises.  If active, she will take clients for walks, to the gym and if given permission by family and doctor will do light training.
  2. Health monitoring: Anita, working with medical professionals, will develop and follow a health plan that is appropriate for the client.  She will  document any changes in the individual’s health, recording and reporting any differences.
  3. Communicating with medical providers:  Anita will coordinate and interact with doctors, nurses and other health providers to ensure a client’s medical needs are being met.  Anita will also oversee medication and prescriptions usage and handle appointment logistics.  She will also communicate with any other caregivers….as some clients need 2 or 3 caregivers for round the clock care.  Anita will oversee the caregivers (even though they work for an agency) when they are in the client’s home, coordinate everyone’s schedules, and keep all family members and care givers on the same page.
  4. Mobility and transportation assistance: Anita can help with getting in and out of a wheelchair, car or shower and driving to and from activities, running errands, and help getting in and out of wheelchair-accessible vehicle.   Anita will also obtain medical supplies and oversee any installations, as needed.
  5.  Emotional support: Anita will always be a stable companion and supporter in all matters personal, health-related and emotional.  Relationships can be the difference-maker in how hard an older person fights against illness or sticks to an exercise and diet regimen.   Additionally, Anita is versed in alternative treatments and can arrange for reiki, art therapy, guided meditation, massage, physical therapy, specialized work out therapies for her clients.  She will arrange for clients (male and female) to get their nails (mani and pedis) done as well as hair styled.
  6.  Home organization: Anita will coordinate with third parties for the organizing, packing or cleaning for a trip, or general house care and cleaning.  Where appropriate, she is also practiced in traveling with clients.
  7.  Meal and nutrition assistance: Anita will insure that there is help and support for meals preparation, shopping, housekeeping, laundry, and other errands.
